CalAIM Initiative Helps Homeless!

Lack of housing and inadequate food are major contributors to health problems. Taking care of those parts of life — social determinants of health, as experts refer to them — could not only improve the lives of patients, but also save money over time. Sierra View Medical Center participates in the new CalAIM initiative by helping those with managed Medi-Cal plans access these new services. Two programs, now in place; Extended Care Management and Medically Tailored Meals are there to help Medi-Cal recipients to access both housing assistance and nutritional food.
