Sierra View Medical Center Seeks Community Council and Committee Members

Sierra View Medical Center is accepting applications for a new Population Health Advisory Council designed to identify and address the unique health care needs of the community. In addition, Sierra View is also seeking members for a new Patient and Family Advisory Committee. This committee will meet monthly to work on the partnerships Sierra View has with the community and how to better quality care for both the patient and their families.
“Sierra View Medical Center’s Patient and Family Advisory Committee (PFAC) l is an exciting step towards achieving the vision of a highly-engaged, patient-centered health care district where patient perspectives will be heard in shaping and improving their health services and care.” Said Dan Blazar, Patient Experience Officer for Sierra View Medical Center.
We are looking for people who are interested in serving on both the PFAC as well as the PHAC to provide our team with constructive advice and feedback that will help improve the health and well-being of those we serve.
Council and Committee requirements include:
- Community members
- Must be over the age of 18 to apply
- Applicants must be committed to building a partnership with Sierra View Medical Center to serve the best interest of the community
- Are able to attend monthly meetings
Community stakeholders will be selected based on position criteria, submission of completed application and recommendation of the internal stakeholders. Community stakeholders will maintain their membership for up to two years.

Once applications have been received, the hospital will review all submissions and begin the formal interview process.
