Because access to your medical information is essential, we will be available to process your requests in person at our Medical Records Office or we can process your requests for release of information remotely. Federal HIPPA laws require an authorization to be completed for all medical record requests unless your provider is requesting them for your care. Because your medical record and the information it contains are confidential, we must confirm your identity before releasing your medical record. Only the patient or his/her legal guardian and/or legal representative can sign the form. You can download the authorization form below.
To obtain your medical records, please bring your valid photo ID or two of the documents listed below with you to the Medical Records department at 396 W. Putnam. We are available to accept medical record requests between the hours of 8 a.m. and 5 p.m. Monday through Friday. You can also fax the request forms with a legible copy of your ID to our fax number at (559) 791- 4826 for processing. We will call you when your records are ready to be picked up.
Please be aware it may take up to 10 days to receive your records.
Our goal is to get your health information to the appropriate healthcare provider as quickly as possible; therefore, we recommend that your physician (or other healthcare provider) request a copy of your medical records to be delivered directly to his/her office by fax.
The Patient Portal provides patients and their families access to health information online at any time and from almost anywhere. With the portal, patients can access their visit history, health summary report, lab results, update your personal information, and more. All of the information in the portal comes from your SVMC Electronic Health Record. This ensures that you have access to the most accurate and up-to-date information.
